Florida Transparency – Financial Information
To accommodate patients requests regarding healthcare treatment, our facility will provide each patient with a personalized estimate of charges and other information prior to the day of service. Please note that the payments and payment ranges are an estimate cost that may be incurred and your actual cost may vary based on actual services rendered. Patients should contact their insurer concerning cost sharing responsibilities.
APPLICATION PROCESS
Riverside Park Surgicenter at this time does not have an application process for any financial assistance programs.
PAYMENT PLAN
The Surgicenter will attempt to contact the patient a minimum of 24 hours prior to admission regarding their estimated financial responsibility (co-pays, deductibles, percentages, etc.) due the day of surgery. Patients are expected to pay their financial obligation. In the event a patient is unable to pay, our Surgicenter may offer a short term repayment schedule after a minimum down payment is made. Patients may pay less for this procedure or service at another facility or in another health care setting.
COLLECTION PROCEDURES
As a courtesy to our patients, we will file an insurance claim to their insurance plan. Patients are expected to pay their financial obligations in a timely manner. The surgery center will attempt to reach a patient by any method available to use to secure payment on an outstanding balance. If the account becomes delinquent, it may be placed with an agency for collection in which their fees and expenses may be the obligation of the patient.
OTHER PROVIDERS
Services may be provided in this health care facility by the facility as well as by other health care providers who may separately bill the patient and who may or may participate with the same health insurers or health maintenance organizations as the facility.
Patients and prospective patients may request from this facility and other health care providers a more personalized estimate of charges and other information. Patients and prospective patients should contact each health care practitioner who will provide services in the ASC to determine the health insurers and health maintenance organizations with which the health care practitioner participates as a network provider or preferred providers.
